Appointments

Appointments can be made during the hours of 8:00am and 4:30pm Monday through Thursday and from 8am until noon on Fridays.  New patients can call our office at (850) 257-7097 or use the contact us page on the website.

 

Spinal Associates provides a spacious, clean and comfortable waiting area that is equipped with a flat screen television and WI-FI access for all patients. Although it is always our goal to maintain an on-time schedule, occasionally an emergency or procedure may take longer than expected which may cause your appointment to be delayed. We always try to inform all patients if a lengthy delay is anticipated and appreciate your cooperation and understanding in these unavoidable situations.

 

Please make sure to arrive 15 minutes early and block out your schedule to make time for your appointment. This time may include filling out paperwork, X-Rays, copying documentation, insurances, etc. New patients coming to the office for the first visit should anticipate extra time. You can shorten your wait time by requesting the new patient paperwork be sent to you by email for printing and completing prior to your appointment.

 

Patients under the age of 18 must be accompanied by their parent or legal guardian. This applies to all appointments.

 

All payments due must be paid at each visit.  For patients who have not met their deductible, please be aware you will be required to pay 100% of the allowable charges for your insurance before they will pay any claims.

 

For your convenience we accept Cash, Check, Visa, MasterCard, American Express and Discover. 

For patients who have been in an auto accident or work related injury, you must contact our office directly and one of our friendly staff members will go through our protocol for these types of appointments.

 

Preparing For Your Appointment

Here is a list of items you will need to have on the day of your appointment:

  • You must bring valid photo identification
  • Your current insurance card
  • Updated prescription list
  • Any medical records you have pertaining to the purpose of your appointment
  • Any X-Ray, CT, or MRI discs and reports pertaining to the area we are evaluating
  • Previous surgery reports (if applicable) pertaining to the area we are evaluating
